Treatment of malignant mesothelioma can be very difficult for a wide variety of reasons. First and foremost, mesothelioma can be difficult to correctly diagnose and may not show up in a patient for decades. This can create problems for Treatment because with mesothelioma, as with all cancers, treatment is more difficult the longer the disease has been allowed to progress. In addition to the difficulties created by delayed treatment of mesothelioma, the disease often does not respond to traditional cancer treatments, further complicating treatment. Also, the organs that are involved in mesothelioma cannot be partially or wholly removed usually, which means that surgical options can be extremely limited. Lastly, the fact that the majority of mesothelioma patients are men of advanced age, usually over 50, means that some more radical treatment approaches cannot be used because of declining health due to old age.
Altogether, this can mean that even mesothelioma patients that have been recently diagnosed can be given a fairly negative prognosis from their doctos. Statistics are hard to come by, but British scientists suggest that 10% of newly diagnosed mesothelioma patients will live for at least three more years; Only 5% will live five years or longer. For patients in the first stage, 50% live for at least two more years. However doctors can be mistaken, and a diagnosis of mesothelioma is in no way always tantamount to a death sentence. In one famous case, scientist Stephen Jay Gould survived with peritoneal mesothelioma for almost twenty years. Eventually, he died from another kind of cancer.
There are four stages of malignant mesothelioma, which measure how far the disease has progressed. How a patient’s mesothelioma is treated depends largely on which stage he or she is in when the disease is found.
* Stage I: Localized mesothelioma that exists only in the lungs, the diaphragm or the pericardial lining.
* Stage II: Advanced mesothelioma that has spread into the lymph nodes of the chest.
* Stage III: Advanced mesotheioma that has spread into the wall of the chest, the center of the chest, the lining of the heart and the diaphragm. Stage III malignant mesothelioma may or may not have spread to the lymph nodes.
* Stage IV: Advanced mesothelioma that has spread far from the chest and abdomen into other organs.

Surgery
Patients with Stage I or milder Stage II mesothelioma are generally offered one or more of the conventional cancer treatments: surgery, radiation and chemotherapy. For early-stage patients, surgery for mesothelioma aims to cure the disease by literally cutting the cancer out of the patient’s body. The most common type of surgery for pleural mesothelioma is a pleurectomy/decortication, which is where doctors remove all or part of the tissues lining the lungs and chest cavity. If doctors find that they can’t remove the cancer without removing the lung underneath those tissues, they may remove one lung as well; this is called a pneumonectomy. A more radical type of surgery for pleural mesothelioma is called an extrapleural pneunonectomy (EPP). In the case of an EPP, surgeons will remove parts of one lung, the pleura, the diaphragm, and the lining of the heart. TUESDAY, April 27 (HealthDay News) — The anticipated approval this week of a therapeutic prostate cancer vaccine by the U.S. Food and Drug Administration could be a milestone against the disease and cancer in general, experts say.
The vaccine, called Provenge, appears to extend survival in men with advanced prostate cancer, and it does so without the serious side effects associated with chemotherapy, radiation and hormone therapy.
“It is certainly exciting to see a drug that has made it this far and appears on the threshold of approval,” said Dr. J. Len Lichtenfeld, the deputy chief medical officer of the American Cancer Society.
The vaccine is not aimed at preventing prostate cancer in men who have not developed the disease, and it is far from a cure for those who have it, Lichtenfeld cautioned. “Provenge represents a modest advance in survival for patients with advanced prostate cancer, but the drug doesn’t delay the progression of the disease,” he said.
Still, it might prove possible to use Provenge in the earlier stages of prostate cancer, where it might be even more effective, Lichtenfeld said. “The hope is if a vaccine is effective in late-stage disease that it is going to be even more effective in the earlier treatment of that same disease,” he said.
But that benefit would have to be shown in clinical trials, Lichtenfeld said.
“If it does get approved it will be groundbreaking.” Lichtenfeld said. “It will represent a new treatment option. It will have a modest impact on prostate cancer survival, but it’s small changes in treatments over time that add up to a major improvement. So I wouldn’t be discouraged by what is a small increment in survival.”
Provenge is a therapeutic (not preventative) vaccine that is made from the patient’s own white blood cells. Once removed from the patient, the cells are treated with the drug and placed back into the patient. These treated cells then cause an immune response, which in turn kills cancer cells, while leaving normal cells unharmed.
Provenge was developed by Seattle-based Dendreon Corp., which conducted initial studies among men with advanced prostate cancer who had already failed standard hormone treatment. Among these men, the vaccine extended life by an average of 4.5 months, although some patients saw their lives extended by two to three years. The only side effects were mild flu-like symptoms, according to the study results.
In 2007, an FDA advisory panel recommended that the agency approve Provenge, but the FDA said more data was needed before it would approve the drug…. (more…)

A $657.5 million settlement reached between the city and 9/11 rescue and cleanup workers on March 11 must be renegotiated, a U.S. District Court Judge said.
Judge Alvin K. Hellerstein rejected the proposed settlement on March 19 and stated, in court, that he did so because the settlement did not offer enough compensation to the workers for the illnesses they have suffered since they worked at or near Ground Zero.
Hellerstein added that he – as opposed to a claims administrator – would supervise any new renegotiation and that the lawyer’s payout, about a third of the settlement, should come out of the same fund that will pay the lawyers for the city, leaving more for the injured worke
“I think the grounds on which [Hellerstein] rejected [the settlement] was strong,” said Daniel Coates, an immigration organizer at the community group Make The Road New York, who has worked with many Queens-area cleanup workers. “But now the process will take longer given the dire situations some of these workers find themselves in.”
According to reports, close to 10,000 workers took part in the lawsuit against the city. These workers claimed that because of the toxins and chemicals – such as cement, glass dust, asbestos, fiberglass, alkaline, and lead – in the air at Ground Zero, they developed respiratory problems including asthma, sinusitis, rhinitis, persistent cough, chronic bronchitis and emphysema, as well as lung scarring, lung cancer, gastrointestinal tract cancer, heart disease and mesothelioma.
As of March 31, 2009, more than 51,000 World Trade Center (WTC) responders nationwide had met eligibility and enrolled in the World Trade Center medical monitoring and treatment programs, according to Fred Blosser, a National Institutes for Occupational Safety and Health (NIOSH) spokesperson.
Some of these workers have had to quit work because they are too sick. As of September 11, 2009, at least 817 had died…. (more…)
